Catastrophic Problem with ADP8866

Right now I'm using an ADP8866 and I'm running into a weird failure mode that causes catastrophic failure of the part over time.  It seems that when the charge pump is activated (1.5x or 2x gain) and we turn on an LED, there is a Huge inrush current (600mA) into the the output of the ADP8866 before the LED turns on.  Do you have any insights into why this may occur?  To me it looks like there’s a latch-up fault and the output capacitance on Vout is getting shorted to ground through the chip.  Additionally this creates a 200mV backpowering spike in the input rail to the ADP8866.
